Electronic Visa Application Forms

Non-Immigrant visa applicants are now required to fill out a specially bar-coded version of the application form, available on the Internet. 

Computer Requirements

To use the electronic visa application forms available below, your Internet browser must support 128-bit encryption, and you should use either an ink-jet or laser printer to print the completed application form. If you are using Internet Explorer (Windows), the minimum version that will work with this site is Version 5.0, with Service Pack 2. If you are using Netscape, the minimum version that will work with this site is Version 6.2. You must also have Adobe Acrobat Reader, Version 5 or higher, installed on your PC in order to download and print the completed application form.

Note: Depending on the speed of your Internet connection, it may take several minutes to download the initial and completed copies of the form. Please be patient.

Instructions

The electronic visa application form or EVAF is available on the State Department website.

1. Enter the information requested into the appropriate spaces on the form. Answer all questions completely. Answers must be in English. Your application may be rejected at the Embassy if each question does not have a complete answer.

2. Review the information for accuracy.  Errors on the EVAF will delay application processing.

3. Press the "Continue" button at the end of the form. Failure to click on the "Continue" button will result in an incorrectly formatted application.

4. Print the form after it is returned to you and displayed in Adobe Acrobat.

5. The barcode printed on the last page contains the information you entered and will help us process your application more quickly.

6. Bring all printed pages with you when you come to the Embassy for your interview.

7. Do not fold the printed application form.

Supplemental Visa Application Forms

Some applicants may need to complete additional forms in connection with their application. These forms are available on the Internet as interactive .pdf files. This format allows applicants to enter all their data on the screen and then print out the form to bring to the visa interview. Please note that these forms do not have bar codes.
